General Description:&nbsp;This grant-funded position will work to empower communities to promote drug free lifestyles; build community structures to support those lifestyles; and encourage and support coordinated, systemic approaches to the prevention of substance use. Position: Community NavigatorMission: UWDOR fights for the health, education and financial stability of every person in our community.&nbsp;Objective: To increase organizational infrastructure and capacity as it directly relates to the provision of the Health-Related Social Needs (HRSN) services as part of the 1115 NYHER Waiver. The mission of HVCC is to create communities of health and wellness for all, which is grounded in a commitment to addressing health disparities. There is increasing recognition that addressing social determinants like food insecurity, housing instability, and lack of transportation significantly improves health outcomes and reduces healthcare costs. To ensure these needs are met for Hudson Valley’s Medicaid Members, the community-based ecosystem of healthcare must consistently serve the whole person, which includes identifies unmet HRSNs needs, connect individuals to appropriate services, and ensure service providers are compensated effectively.The Community Navigator will:<ul><li>Strengthen the organization’s overall capacity to effectively provide HRSN services in the community</li><li>Increase program delivery outcomes and skill-based projects</li><li>Implement targeted capacity-building initiatives</li><li>Expand service delivery capacity including the # of screens</li><li>Track data and report outcomes quarterly</li><li>Increase the number of Medicaid members screened for community services</li><li>Conduct extensive community outreach to community members</li><li>Meet expected performance measures</li><li>Participate in technical assistance and training opportunities</li><li>Work with community partners around shared objectives</li><li>Reach and connect services to Medicaid eligible community members</li><li>Collaborate and liaise with SCN staff</li><li>Other directives as required</li></ul>Requirements: Associates degree and/or 2 years of community outreach experience.
