POSITION: PARENTS AS TEACHERS CASE MANAGER 
 

REPORTS TO: Director of Early Childhood Initiatives 
 
Purpose: The Case Manager contributes to the achievement of United 

Way of El Paso County’s (UWEPC) mission, vision, and core 
values by addressing a key strategy: family home visitation 
programs. The Case Manager is responsible for providing 
needs assessment, service planning, and monitoring and 
evaluation of case management services to children and 
families enrolled in the Parents as Teachers Program, an 
evidence-based early childhood home visitation framework 
that builds strong communities, thriving families and children 
who are healthy, safe and ready to learn. 

 
MAJOR DUTIES &amp; RESPONSIBILITIES: 
The Parents as Teachers (PAT) Case Manager Responsibilities include: 

ï· Complete intake record and identify reasons for referral. 
ï· Assist participants enrolled in the PAT Program with basic needs. 
ï· Create and customize a service plan with each participant receiving case 

management services. 
ï· Monitor and evaluate the service plan as needed during services period. 
ï· Be in contact with the family monthly until terms on the service plan are 

addressed/provided/remedied. 
ï· Complete warm-hands off/referrals to entities in El Paso that are better 

suited/preferred to meet the needs of the PAT participants. 

 
Administrative Compliance for HOPES: 

ï· Complete data entry in data entry systems. 
ï· Maintain accurate and complete family file records. 
ï· Complete daily timesheets, staff weekly and monthly reports, monthly 

mileage and expense reports by set deadlines. 
ï· Participate in monthly reflective and administrative supervision sessions. 
ï· Recruit families to maintain a full caseload by conducting presentations in 

small or large groups. 
ï· Request donations and incentives for active participants in the PAT 

program. 
 


Professional Development 
ï· Complete at least 20 hours of professional development each year. 
ï· Attend all grant-required trainings.

Qualifications--Knowledge, Education and Experience 
 
Must have a minimum of a bachelor's degree in Social Work or closely related 
field (counseling, psychology, etc.) 
 
Must have at least 3 years of hands-on experience working with families and 
children. Bilingual (English/Spanish) is required. 
 
The ability to extend emotional support to families in crisis with a non-judgmental 
attitude, offering compassion and strength in an objective manner. 
 
Leadership qualities to include the ability to give direction and to work 
cooperatively with peers and accept direction as needed. 
 
Knowledge of the non-profit sector is essential. Knowledge of community 
resources and ability to access needed services for families.

Essential Duties and Responsibilities include the following 

 • Travel to meet with clients at their home, community partner offices, satellite offices, etc.; if 
necessary, to ensure at-risk client’s access to services 

 • If an emergency situation, determine the appropriate type of financial assistance necessary to 
stabilize the client’s emergency situation 

 • Maintain detailed case records, complete required program documentation, and input timely case 
data into Apricot database 

 • Ensure data integrity throughout the client tracking process to confirm service referrals within 
UWSHR; and outside to community partners, are completed and customers are receiving all the 
services they need 

 • Process payments in SPENDwise, to ensure client’s emergency financial needs are addressed in 
a timely manner 

 • Processes data for monthly and quarterly reports, and as needed 

 • Carries out Other Duties as Assigned 

 • Ability to properly handle and manage confidential data 

 • Serve as a key member of the NRHA team to maintain and implement core 

program framework/changes, and strategies.

 • Associate’s degree and/or equivalent experience 

 • HUD Certified Housing Counselor a plus but not required 

 • 1-2 years of experience in human services programs requiring direct client interactions required

needs of Housing Choice Voucher Program participants and the ALICE population

goal achievement, and referrals within specified guidelines.

 • Maintain a working knowledge of community resources and advocacy to initiate and coordinate service delivery and 
follow-up.

United Way of Greater Cincinnati (UWGC) is seeking a detail-oriented Associate to join our growing Project LIFT Network team. Project LIFT is a community-driven initiative focused on empowering families financially through collaborative efforts with diverse partners across the region.This role serves as a central support within the Project LIFT Network to manage successful network operations, ensure quality data collection and reporting, and promote effective process and process improvement.&nbsp;General Function:<ul><li>Facilitate Effective Network Processes: Collaborate with the Senior Manager to ensure alignment and connectivity among Project LIFT Network partners. Promote understanding of the network's shared purpose and goals and create efficient processes to support network activities.</li></ul><ul><li>Facilitate Data Management and Process Improvement: Develop and maintain processes and procedures to support the coordination of network activities and functions. Ensure data quality through data collection, audits, and continuous improvement efforts. Support the implementation of new technology platforms and tools to enhance network efficiency.</li></ul><ul><li>Guide Network Activities: Provide guidance to network members on data collection, analysis, and reporting.
