Purpose of PositionThis position supports the needs and activities of the Marketing and Communications team. The 211 Resource Navigator plays a vital role in helping residents access social services by collecting data, conducting needs assessments and facilitating referrals within a network of community partners. This role is challenging, emotionally demanding and deeply impactful. Resource Navigators provide a broad-based human services assessment, making appropriate service referrals through a referral platform and ensuring follow-up to confirm referrals are utilized effectively. The 211 Resource Navigator’s responsibilities may evolve to align with new community needs and United Way initiatives, ensuring flexible and responsive support to residents. All calls to 211 are recorded for quality and training purposes. KEY AREAS OF RESPONSIBILITY:<ul><li>Data Tracking &amp; Advocacy: Secure consent and build caller profiles for longitudinal records. Collect and record data on referrals, including types, demographics, and agency responses, to contribute to daily and weekly reports. Act as an advocate for residents, ensuring they are connected to the resources they need.</li><li>Client Support &amp; Referrals: Compassionately assess the needs of community members and provide referrals for immediate and ongoing support through our network of trusted community partners. Coordinator, Resource Development (Highlands County, FL) 

The 211 Care Coordinator assists residents in connecting to social service support by assessing needs and making referrals within a network of partnering organizations. The Care Coordinator will provide a broad-based human services needs assessment; make referrals within the referral platform; collect and record appropriate data on referrals; and follow up to ensure referrals are utilized and complete. The Care Coordination function is flexible and responsive to changing community needs. As such, the Coordinator’s focus may change to meet new external and United Way opportunities, initiatives, and programs to support community members.
Key Areas of Responsibility: 
<ul><li>Assist community members by assessing needs in a compassionate method and providing referrals for immediate and intermediate needs through a network of key community partners. Referrals will be made primarily through a technology platform. The Care Coordinator is responsible for tracking when partners accept or decline referrals, when agencies contact residents, and when services are provided. Position Reports to:&nbsp; President &amp; CEO of United Way of Greater Plymouth County
Work Schedule:&nbsp; Monday through Friday work week schedule with some evenings and weekends.&nbsp; Flexibility is available as warranted.

Summary:&nbsp; Acts as a proactive liaison to United Way, the community, and partners by promoting support, positive communication, and outreach. Participates in community needs assessments and allocations processes to ensure the effectiveness of programs that address critical needs in the community.&nbsp; Supports the recruitment and active work of volunteers to assist with the mission of the organization.&nbsp; Represents and communicates United Way in a highly visible, positive manner as a collaborative, impactful and trustworthy organization. Maintains regular contact with United Way staff, and the Board of Directors as well as with donors, community partners, and other community stakeholders to provide information and guidance to maximize United Way’s visibility, viability, and community impact.
Essential Functions and Accountabilities: 
Community Relations – 35%
<ul><li>Directly coordinates and facilitates specific community coalitions, including but not limited to Brockton &amp; Plymouth Community Health Network Areas.</li>
<li>Promotes strong relationships with both member and non-member community health and human service organizations through positive communication and attendance at meetings and events.</li>

Responsible for keeping accurate and organized records of all community impact activities in the Andar system, annual reports for United Way Worldwide, including Database 2, annual budgeting, and annual audit reports.
