POSITION SUMMARY Provide individuals with suicide intervention, safety planning, crisis de-escalation, problem solving, brief assessment and appropriate social service referrals within a contact center environment. Manager, Community Partnership School Gulfstream

General Description: &nbsp;This position is responsible for coordinating, facilitating, and tracking the school healthcare referral network and onsite services within a Community Partnership School™ at Hallandale Magnet High School. Health services will include primary care, dental, vision, and behavioral health for youth and adults.&nbsp;Primary Job Responsibilities:<ol><li>Facilitate relationships with local healthcare providers to build and maintain on campus health services for students, staff, and families&nbsp;at Hallandale Magnet High School.</li><li>Coordinate and track the&nbsp;Community Partnership School (CPS) healthcare referral network and processes in Hallandale Beach.&nbsp;</li><li>Serve as the point of contact between CPS administration, school nurse, medical clinic, behavioral health department, and other health-related agencies/departments providing services.</li><li>Serve as entry point to the Community Partnership School for students and parents.</li></ol>Secondary Job Duties:<ol><li>Establish referral network/linkages with outside agencies.</li><li>Match all medical clinic referrals, including health, immunization, dental, vision, and behavioral health referrals.</li><li>Track and establish follow up procedures for CPS referrals made in and out of school.</li><li>Outreach through phone call or written communication to identify individual needs and program eligibility.</li><li>Participate in school Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.</li><li>Coordinate holistic health and wellness events for students, staff &amp;
