POSITION SUMMARY Dentist’s responsibilities include developing suitable treatment plans for patients, keeping an accurate record of all appointments, diagnoses, and treatments, and answering patients’ questions. ESSENTIAL DUTIES &amp; RESPONSIBILITIES 1. To participate in staff meetings and training programs as directed and to the extent necessary to maintain currency of credentials and to assure knowledge and skills necessary for required professional practice.5. Position SummaryThe Case Manager will play a central role in implementing the Integrated Patient Care Program by overseeing frontline care coordination activities. Reporting to the Program Manager, the Case Manager will directly supervise Social Workers and Community Health Workers and work closely with partner agencies to ensure patients receive coordinated medical, dental, vision, and social support services.
