Job Title:&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Post Eviction Case ManagerReporting to:&nbsp;Community Impact ManagerDepartment:&nbsp;Community Impact and Operations&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Overall Purpose of this Role:The Post Eviction Case Manager functions to support United Ways’ collective impact work of providing advocacy and referral services.&nbsp; This advocate will help individuals who have been evicted or are at risk of homelessness by providing resources, advocacy, support and planning. Their role is to assist clients in securing stable housing when available, accessing community and social services, and navigating legal and financial challenges related to the eviction, and developing strategies for safe transitions. We commit to building bridges across cultural, racial, religious, and economic boundaries.OVERVIEW OF RESPONSIBILITIESThe Parent Educator contributes to the achievement of United Way of El Paso County’s (UWEPC) mission, vision, and Education Community Impact goals by addressing a key strategy: family home visitation programs.&nbsp; The Program Manger’s responsibility is to successfully deliver the Parents as Teachers program, an evidence-based early childhood home visitation framework that builds strong communities, thriving families and children who are healthy, safe and ready to learn.KEY RESPONSIBILITIES/ESSENTIAL FUNCTIONSThe Parents as Teachers (PAT) Parent Educator delivers the essential PAT program functions, to include:<ul><li>Preparing for and conducting monthly or bi-monthly home visits, including home visits with reoccurring access to vulnerable populations, which will require 27 to 35 visits per month</li><li>Partner with families to improve well-being, reflect with families regarding parenting styles, and facilitate discussions and play activities to foster healthy development</li><li>Planning and conducting group connections and family appreciation events</li><li>Conducting screenings and assessments for parents and children</li><li>Utilizing identified resources available to provide family support</li></ul>Administrative Compliance for Parents as Teachers<ul><li>Complete data entry in two electronic databases including Penelope (PAT) and Extended Reach.</li><li>Maintain accurate and complete family file records.</li><li>Complete daily timesheets, staff weekly and monthly reports, monthly mileage and expense reports by set deadlines.</li><li>Participate in monthly reflective and administrative supervision sessions.</li><li>Recruit families to maintain a full caseload by conducting presentations in small or large groups.</li><li>Request donations and incentives for active participants in the Parents as Teachers (PAT) program.</li></ul>Professional Development<ul><li>Complete at least 20 hours of professional development each year to maintain Parents as Teachers credential.</li><li>Attend all required trainings: Core Curriculum trainings, Technology Training, PAT Foundational and Model Implementation trainings, Ages and Stages Questionnaire, Life Skills Progression, Cultural Competency Training, Health Screenings and other trainings as recommended by program to enhance professional skills and capacity building.</li></ul>EDUCATION/REQUIREMENTSMust have a bachelor’s degree in a health and human services field or early childhood education.&nbsp;Must have at least two (2) years of direct service experience in a health and human services field and in working with families and children.Must befluent in English and Spanish.Successful candidate must also embody personal attributes of honesty, integrity, professional behavior, tact, sense of urgency, and sense of humor.Must possess a valid driver’s license, current auto insurance, and daily access to reliable vehicle.Must be able to pass a background check.The above statements are intended to describe the general nature and level of work being performed by the Parent Educator.&nbsp; They are not intended to be an exhaustive list of all duties, responsibilities, and skills required.Must be able to work a flexible schedule with the possibility of early morning, evening, and weekend work.The physical demands described here are representative of those that must be met by an employee.
