The Community Engagement and Programs Coordinator is an engaged member of the Community Impact and Engagement Team who is results-oriented and detail focused. Key Accountabilities 

ï· Travel to meet with clients at their home, community partner offices, satellite offices, etc.; if
necessary, to ensure at-risk client’s access to services

participants/families on their journey towards health, wellness, and self-

ï· Follow up with participants via phone calls, home visits and visits to other settings where participants
can be found.
