Position Summary

The Program Manager will lead the day-to-day management and implementation of the Integrated Patient Care Program funded by The Duke Endowment. Reporting to the WellPartners Executive Director, this role provides overall program leadership, coordination, and accountability across partner agencies. The Program Manager will oversee case manager and program staff and will serve as the primary point of contact for external partners and funders.

This position requires strong experience in program management, grants management, operations, and cross-sector collaboration, with a focus on integrating care, addressing social determinants of health, and improving patient outcomes.

Key Responsibilities

Program Leadership & Management

  • Lead daily operations of the Integrated Patient Care Program, ensuring alignment with grant objectives, timelines, and outcomes.
  • Oversee program staff, case management functions, social workers, community health workers, interns, and contractors.
  • Serve as the primary liaison to partner organizations, including Lexington Health, United Way of the Midlands, Good Samaritan Clinic, and The Free Medical Clinic.
  • Coordinate implementation of common intake, referral, and patient education processes across partner clinics.

Grants Management & Compliance

  • Manage grant reporting, documentation, and compliance requirements in coordination with WellPartners leadership and fiscal partners.
  • Track program deliverables, outputs, and outcomes in alignment with Duke Endowment reporting standards.
  • Support sustainability planning, including diversification of funding and preparation for future grant opportunities.

Operations & Continuous Improvement

  • Establish and maintain program workflows, performance metrics, and feedback loops.
  • Facilitate monthly partner meetings and quarterly outcome reviews.
  • Oversee development of program documentation and replication materials for use in other communities.

Strategic & External Engagement

  • Represent WellPartners in partner, funder, and stakeholder meetings related to integrated care.
  • Support fundraising and partnership development efforts tied to program sustainability and growth.
  • Collaborate with senior leadership to align program activities with WellPartners’ broader strategic goals.

Qualifications

  • Bachelor’s degree required; Master’s degree in Public Health, Social Work, Nonprofit Management, or related field preferred.
  • Minimum 5–7 years of experience in program management, preferably in healthcare, social services, or nonprofit settings.
  • Demonstrated experience managing grant-funded programs and cross-organizational partnerships.
  • Strong operational, organizational, and communication skills.
  • Experience supervising multidisciplinary teams.
  • Knowledge of social determinants of health and integrated care models strongly preferred.
