Parents as Teachers Case Manager

POSITION: PARENTS AS TEACHERS CASE MANAGER
REPORTS TO: Director of Early Childhood Initiatives Purpose: The Case Manager contributes to the achievement of United
Way of El Paso County’s (UWEPC) mission, vision, and core values by addressing a key strategy: home visitation programs. The Case Manager is responsible for providing needs assessment, service planning, and monitoring and evaluation of case management services to children and families enrolled in the Parents as Teachers Program, an evidence-based early childhood home visitation framework that builds strong communities, thriving families and children who are healthy, safe and ready to learn.
MAJOR DUTIES & RESPONSIBILITIES: The Parents as Teachers (PAT) Case Manager Responsibilities include:
ï· Complete intake record and identify reasons for referral. ï· Assist participants enrolled in the PAT Program with basic needs. ï· Create and customize a service plan with each participant receiving case
management services. ï· Monitor and evaluate the service plan as needed during services period. ï· Be in contact with the family monthly until terms on the service plan are
addressed/provided/remedied. ï· Complete warm-hands off/referrals to entities in El Paso that are better
suited/preferred to meet the needs of the PAT participants.
Administrative Compliance for HOPES:
ï· Complete data entry in data entry systems. ï· Maintain accurate and complete family file records. ï· Complete daily timesheets, staff weekly and monthly reports, monthly
mileage and expense reports by set deadlines. ï· Participate in monthly reflective and administrative supervision sessions. ï· Recruit families to maintain a full caseload by conducting presentations in
small or large groups.

ï· Request donations and incentives for active participants in the PAT program.
Professional Development ï· Complete at least 20 hours of professional development each year. ï· Attend all grant-required trainings.
CROSS-FUNCTIONAL RESPONSIBILITIES: Community Engagement
ï· Work effectively with other teams at United Way to provide appropriate support and value to partners. Leverage relationships to create additional opportunities with other organizations and individuals.
ï· Foster creative thinking and facilitate future partnerships within corporate accounts.
ï· Attend key community events and activities as a way to promote United Way of El Paso County’s mission, vision and core values becoming a defined community ambassador for United Way.
ï· Cultivate community support for United Way by maintaining contacts with local organizations, businesses and individuals.

Qualifications--Knowledge, Education and Experience Bachelor’s degree in subjects related to health and human services, education, health, sociology or related field is required. An equivalent combination of education, experience and skills and abilities will be considered. Bilingual (English/Spanish) is required. The ability to extend emotional support to families in crisis with a non-judgmental attitude, offering compassion and strength in an objective manner. Leadership qualities to include the ability to give direction and to work cooperatively with peers and accept direction as needed. Knowledge of the non-profit sector is essential. Knowledge of community resources and ability to access needed services for families. Successful candidate must also embody personal attributes of honesty, integrity, professional behavior, tact, sense of urgency, and sense of humor. Must be able to work weekends and have the flexible schedule for early morning and evening work. The physical demands described here are representative of those that must be met by an employee.
