Position Summary

WellPartners Dental & Eye Health is seeking an experienced, collaborative leader to serve as the Director, Integrated Patient Care Initiative. This position will lead the implementation and ongoing operations of WellPartners' Integrated Patient Care (IPC) Initiative, a transformational, multi-year program funded by The Duke Endowment.

WellPartners Dental & Eye Health is an independent nonprofit organization providing comprehensive dental, vision, and integrated care services to uninsured and underserved individuals throughout South Carolina's Midlands.

Reporting directly to the Executive Director, the Director serves as a member of the WellPartners Leadership Team and is responsible for leading the Integrated Patient Care Initiative while helping advance organizational priorities, cross-functional collaboration, and strategic growth.

The Director will oversee day-to-day program operations, supervise Community Health Workers and other program staff, develop community partnerships, coordinate patient navigation services, monitor grant outcomes, and build systems that improve access to coordinated, whole-person care for uninsured and underserved patients.

This position offers a unique opportunity to help shape an innovative model of integrated care that connects dental, vision, medical, behavioral health, and community resources to improve health outcomes across the Midlands.

Essential Responsibilities

Program Leadership & Operations

  • Provide strategic and operational leadership for the implementation, growth, and long-term sustainability of the Integrated Patient Care Initiative
  • Develop operational workflows, policies, and procedures supporting integrated care.
  • Coordinate program activities across WellPartners' dental and vision clinics.
  • Identify opportunities to improve efficiency, patient experience, and clinical collaboration.
  • Support future expansion of integrated care services.

Team Leadership

  • Supervise Community Health Workers and future program staff.
  • Provide coaching, performance management, and professional development.
  • Coordinate onboarding and supervision of MSW interns, AmeriCorps members, and other trainees as appropriate.
  • Foster a collaborative, patient-centered team culture.

Care Coordination

  • Oversee patient navigation and referral processes.
  • Strengthen coordination between medical, behavioral health, dental, vision, and community partners.
  • Ensure timely follow-up on referrals and patient engagement activities.
  • Promote whole-person care and address social determinants of health.

Community Partnerships

  • Build and maintain strong relationships with healthcare providers, nonprofit organizations, educational institutions, and community agencies.
  • Represent WellPartners in collaborative meetings and community initiatives.
  • Identify opportunities to expand partnerships that improve patient access and outcomes.

Data, Reporting & Quality Improvement

  • Monitor program performance and patient outcomes.
  • Develop dashboards and reports for leadership and funders.
  • Ensure compliance with Duke Endowment grant requirements.
  • Use data to drive continuous quality improvement and operational decision-making.

Organizational Leadership

  • Act on behalf of the Executive Director, as appropriate, in cross-functional initiatives and represent WellPartners with external partners, funders, and community stakeholders.
  • Serve as a member of the WellPartners Leadership Team, participating in organizational planning, operational decision-making, and strategic initiatives that extend beyond the Integrated Patient Care Initiative.
  • Collaborate across departments to strengthen organizational effectiveness and advance WellPartners' mission.
  • Support grant development, strategic planning, and organizational growth initiatives as assigned by the Executive Director.Qualifications
  • Required

  • Bachelor's degree in Public Health, Healthcare Administration, Social Work, Nursing, Human Services, Business Administration, or a related field.
  • Three to five years of progressively responsible leadership experience in healthcare, nonprofit management, care coordination, public health, or human services.
  • Experience leading programs or projects involving multiple stakeholders.
  • Experience supervising staff or multidisciplinary teams.
  • Strong organizational, communication, and project management skills.
  • Proficiency with Microsoft Office and data management systems.
  • Preferred

  • Master's degree in a related field.
  • Experience managing grant-funded programs.
  • Experience collaborating with Community Health Workers, care coordination, or patient navigation programs.
  • Knowledge of integrated care models and social determinants of health.
  • Experience serving underserved or uninsured populations.

  • What Success Looks Like in the First 12 Months

  • The Director will play a significant role in establishing WellPartners' integrated care model. Success during the first year will include:
  • Successfully recruit, onboard, supervise, and develop two Community Health Workers.
  • Implement standardized workflows for patient navigation, care coordination, and referrals across WellPartners' dental and vision clinics.
  • Strengthen partnerships with healthcare providers, behavioral health organizations, social service agencies, educational institutions, and community organizations.
  • Develop meaningful performance dashboards and outcome measures that demonstrate patient impact and support continuous quality improvement.
  • Successfully meet all Duke Endowment grant deliverables, reporting requirements, and compliance expectations.
  • Build sustainable operational systems that position the Integrated Patient Care Initiative for long-term growth and future funding opportunities.
  • Become a trusted strategic advisor to the Executive Director, provide operational leadership, strengthen organizational capacity, and help position WellPartners for long-term growth and sustainability.
