Position Overview
We’re seeking a compassionate and dedicated Community Care Navigator to serve Highlands and Hardee County residents by connecting them to vital health and human services. This includes resources for food, shelter, transportation, childcare, financial assistance, and more -- based on each individual’s or family’s unique needs.
Key Responsibilities
- Build awareness of available benefits and social services for Highlands and Hardee County residents, making referrals to appropriate programs such as SNAP, Medicaid, KidCare, and food pantries.
- Actively engage with the community through events, meetings, and outreach efforts to connect with underserved populations.
- Travel throughout Highlands and Hardee counties to promote resources, build relationships, and support residents.
- Follow up with individuals and families to ensure needs are being addressed.
- Document all interactions and event data accurately in tracking systems.
- Support departmental goals through proactive teamwork and continuous learning.
- Demonstrate leadership through collaboration and knowledge-sharing.

- Experience working with diverse and multicultural populations.
- Proficiency in Microsoft Office (especially Word and Excel); familiarity with SharePoint preferred.
Ideal Candidate Profile
- Associate or bachelor’s degree in health and human services or a related field (preferred), or equivalent community engagement experience.
- Background in sales, outreach, or community relations a plus.
- Strong time-management skills and interpersonal communication.
- Bilingual English/Spanish preferred.
Additional Requirements
This role requires regular travel across Highlands and Hardee counties and participation in outdoor events in Florida’s climate. When not in the field, in-office presence at UWCF’s Highland City location is expected. Candidates must have reliable transportation, a valid Florida driver’s license, and the ability to attend off-site meetings, events, and worksites.
Compensation and Benefits
This is a temporary, 1-year, grant-funded, full-time, hourly role with a starting rate of $25 per hour.
