This role entails direct communication with partner organizations to ensure accurate information for 211 callers and participating in community outreach events to build relationships and gather information. This role includes responding to 211 requests through phone, text, in-person, and email and tracking data on referrals and service navigation efforts for UWNU. Strong organizational, communication, and interpersonal skills are essential, and experience in community outreach is preferred.Key Responsibilities:211 Database Management<ul><li>Monitor and continuously update highly-used community services by collaborating with local providers to ensure they are portrayed accurately in the 211 database and website. Case Management 

ï· Provision of holistic and comprehensive assessment, case management, crisis 
intervention and behavioral health services to clients including but not limited to 
intake assessment and ongoing problem-solving. 

ï· Provide referrals and resources to community members seeking support as a 
result of the tragic shooting on August 3, 2019. 

ï· Provide a warm hand-off for community members when making referrals to other 
agencies or partner programs. Follow up with agencies and clients, as

continually provide feedback to the Director on the need for evolving programs

The Parents as Teachers (PAT) Case Manager Responsibilities include:

ï· Assist participants enrolled in the PAT Program with basic needs.

ï· Monitor and evaluate the service plan as needed during services period.

Qualifications--Knowledge, Education and Experience
