General Description: &nbsp;This position is responsible for coordinating, facilitating, and tracking the school healthcare referral network and onsite services within a Community Partnership School™ at Hallandale Magnet High School. Health services will include primary care, dental, vision, and behavioral health for youth and adults.&nbsp;Primary Job Responsibilities:<ol><li>Facilitate relationships with local healthcare providers to build and maintain on campus health services for students, staff, and families&nbsp;at Hallandale Magnet High School.</li><li>Coordinate and track the&nbsp;Community Partnership School (CPS) healthcare referral network and processes in Hallandale Beach.&nbsp;</li><li>Serve as the point of contact between CPS administration, school nurse, medical clinic, behavioral health department, and other health-related agencies/departments providing services.</li><li>Serve as entry point to the Community Partnership School for students and parents.</li></ol>Secondary Job Duties:<ol><li>Establish referral network/linkages with outside agencies.</li><li>Match all medical clinic referrals, including health, immunization, dental, vision, and behavioral health referrals.</li><li>Track and establish follow up procedures for CPS referrals made in and out of school.</li><li>Outreach through phone call or written communication to identify individual needs and program eligibility.</li><li>Participate in school Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.</li><li>Coordinate holistic health and wellness events for students, staff &amp; PURPOSE OF POSITION: Reporting directly to the CEO, the primary objective of the Chief 

Development Officer (CDO) is to increase revenue in a strategic manner that drives the mission 

of the United Way of Forsyth County. The CDO will lead the development and implementation 

of fundraising strategies that ensure organizational capacity to deliver on the ambitious goals of 

our strategic plan. The CDO ensures the effective execution and growth of the organization’s 

revenue generation through the engagement of donors and volunteers and the coordination of 

executive volunteer and staff efforts related to revenue generation activities. The CDO will 

work collaboratively with the Chief Marketing Officer who will lead planning and execution of 

all major events. 
 

DUTIES AND RESPONSIBILITIES: 
 

1. Leads the organization’s revenue generation efforts. Responsible for generating a 

long-term strategic growth vision and developing and executing Board of Directors 

strategic plan in relation to revenue generation. The CDO is responsible for:

Position Summary: 


The Donor Relations Manager develops plans, implements and evaluates 

accounts of United Way of Forsyth County corporate partners and individuals 

with a focus on identifying growth opportunities as well as identifying new 

resource opportunities in order to enhance annual and long-term fundraising 

goals and promote UWFC’s strategic mission, with a measured focus on 

personal donor connections.

Position Duties &amp; Responsibilities

ï· Prepares and executes year-round relationship plans for company 

accounts and individual investors with a focus on maximizing potential 

and growth. 

ï· Develop and adapt plans for challenging companies 

ï· Focus on attracting and developing new partnerships. 

ï· Heavy focus on individual donor engagement with expectations of 

meeting ambitious goals around personal contact with donors and 

prospects. 

ï· Works closely with volunteers to leverage experience and connections 

to gain better access to company leadership and build strong, more 

effective relationships. 

ï· Prepares and executes an ongoing engagement plan for assigned 

individual donors. 

ï· Cultivate major gifts and planned giving prospects through ongoing 

engagement. 

ï· Prepares and executes an individual campaign plan for all accounts 

assigned. 

ï· Ensures the accuracy of data on all accounts and maintains strict 

confidentiality of personal account and financial information. 

ï· Works as part of a team to develop and execute a year-round 

engagement plan for companies and individual investors. 

ï· Establishes rigorous individualized personal and professional 

development plans. 


ï· Understands and supports UWFC’s strategic direction in the 

community. 

ï· Identifies and develops new opportunities for resources. (Expansion in 

existing accounts as well as potential new companies/investors) 

ï· A willingness to take on additional duties to ensure the success of the 

team 

ï· Occasional local travel may be required with some events/tasks outside 

of traditional work hours

20lbs. 

ï· NC Driver’s license and auto insurance are required. 

ï· This is a hybrid position; 4 days in the office Friday remote. 

ï· FLSA status: Exempt
