Position Title: Family Support Coach, Bridging the Gap
Position Term: This position is slated to run through 12/24/2023
Status: Part-time, expected to work 24 hours/week. Non-exempt.
Salary: $21.15/hour.

Location: Primarily based of our Denver office at 711 Park Ave W, Denver, CO 80205, and frequently traveling to local offsite meetings. Ability to work remote 2 days/week, depending on time of year.

How You'll Help
The Family Support Coach, Bridging the Gap will employ a strengths-based approach to support young people ages 18 and older experiencing homelessness, and/or exiting the foster care system and Division of Youth Services. The Family Support Coach will provide one-on-one support, resources, and opportunities to program participants as they identify and access critical basic needs and services related to their transition to economic and financial stability, including transitional housing vouchers. The Family Support Coach will also help with the coordination of wrap around services with Adams County partners.
Essential Responsibilities
- Provide case management to youth from Adams County who have Foster Youth Independence vouchers.
- Establish partnerships with community organizations listed on grant.
- Build and establish relationship relationships with participants (and their families) in the Bridging the Gap (BTG) program.
- Facilitate the development of both formal and informal supports for each of their participants. The Family Support Coach (FSC) will engage young adults with programs, services, and opportunities within the following four domains: personal growth including workforce and education, physical and mental health, financial health and social well-being and engagement.
- Utilizing a trauma-informed coaching model, the FSC will support participants as they develop a vision of what they would like to achieve within the four domain areas. Through the co-creation of a participant led service plan that supports the participants understanding and practice of behaviors and skills that can develop confidence, promote independence, and establish stability. The FSC will collaborate with and assist participants in learning the skills to advocate for themselves.
- Connect with CCR team to help participants identify opportunities to discover their strengths and interests and to explore pathways to education and employment. Collaborate with CCR team to provide guidance and hands on support related to employment, job readiness (resume writing, job leads, interviewing preparation, etc.) as well as high school graduation, GED completion and post-secondary pursuits (FAFSA, scholarships, applications, etc.).
- Collaborate with Housing team to ensure participants are meeting goals and are on the path to achieve their goals
- Provide support and guidance, to participants as they navigate challenging situations.
- Support participants as they identify, access, and navigate systems designed to support their transition to living independently including obtaining medical coverage as well as forms of public assistance (TANF, SNAP, CCAP, bus passes, etc.).
- Coordinate with Bridging the Gap (BTG) staff on the planning, marketing/recruitment, and implementation of BTG events and activities as it relates to family and advocacy supports.
- Responsible for documenting accurate participant information and activities in the database system, following client confidentiality, database security protocols and data integrity.
What You'll Need: Education, Experience, and Skills
- High School Diploma/GED required; Associates degree Bachelor's degree preferred or equivalent experience.
- Minimum 1-3 years of experience working with youth or young adults in the following sectors: mental health, school districts, juvenile justice, law enforcement and human service agencies, and a minimum 3 years related experience providing supportive services to underserved communities.
- Demonstrated experience working with diverse populations
- Strong understanding of youth development factors
- Strong ties and understanding of community resources
- Strong case management skills
- Excellent communication skills
- Strong organizational skills
- Creative and flexible nature
- Must also successfully meet and maintain Mile High United Way's driving criteria.
- Must possess and maintain a valid driver's license.
- Reliable transportation is required as you will be utilizing your own personal vehicle for visiting youth's homes and conducting coaching sessions in the community. You will be reimbursed for mileage.
Working Environment: 60% of time in typical office environment, 40% traveling to outside meetings and inspections. Must also successfully meet and maintain Mile High United Way's driving criteria. Must possess and maintain a valid driver's license.
Computer Skills: Proficient in working with Microsoft Office and Salesforce.
Physical Activities: Must have current CO driver's license, applicable insurance, and a safe driving record. Ability to lift 20lbs
