POSITION SUMMARY
We connect people to resources. The Care Coordinator (CC) will be helping with the daily operations of the Utah Coordination Center. The CC will be key in connecting referrals to essential resources and regularly communicating with partnering organizations. The position will be responsible for responding, in a timely manner, to digital referral requests received through a web-based platform from community-based organizations (CBOs) participating in the Unite Utah Network – a coordinated care referral network. The CC position will work in collaboration with United Way of Utah County (UWUC), United Way of Northern Utah (UWNU) and other United Ways in Utah to ensure statewide alignment. This position will mostly require work between regular office hours Monday – Friday from 8 am – 6 pm. United Way of Salt Lake is a hybrid workplace.

ESSENTIAL FUNCTIONS
Oversee service quality in the work of the operations team and leads projects pertaining to service delivery. Create and provide onboarding and other training and supports the Connection Center Manager in their duties of leading the operations team.

-Support the day-to-day operations of the Utah Coordination Center.
-Coordinate client care with providers inside the network; request updates on client cases from network providers; ensure services are provided to clients when appropriate.
-Maintain an in-depth understanding of network capabilities to ensure high-quality referrals.
-Navigate systems on behalf of clients and identify necessary resources at the local and state level.
-Be accessible via the Unite Us web platform and telephony systems to obtain or provide client status updates and facilitate timely referrals.
-Review and assign incoming and outgoing referrals or assistance requests to the appropriate service provider(s).
-Capture a balanced perspective of both client and provider experiences and identify gaps in services available through the network.
-Use feedback from clients and providers to streamline and improve network operations.
-Navigate multiple databases to access appropriate information to assist each client.
-Build and maintain relationships with partners.
-Collaborate with other 211 teams to achieve program expectations.
-Communicate in a professional manner, written and verbally, about aspects and outcomes of 211’s body of work to colleagues, leadership, and stakeholders.
-Participate in Utah 211 staff meetings, trainings, and partner meetings as requested by the leadership and contract partners.
-Provide additional support and resources as requested during disaster activation.
-Other duties as assigned.

BENEFITS
This is a non-exempt, full-time position with full benefits. $38,000-49,000. Wages commensurate with experience.

REQUIRED COMPETENCIES
-Strong interpersonal skills to communicate and work effectively with individuals of diverse backgrounds, needs, and communication styles.
-Driven with a strong work ethic. Ability to work independently and stay on task to achieve short- and long-term goals.
-Ability to maintain confidential information.
-Trauma Informed Care competent.
-Professional and/or personal experience with and knowledge of health and human services resources in Utah, and the needs and challenges faced by vulnerable populations across the state.
-Ability to use office equipment and automated systems/software, including database experience.
-Ability to work in a team environment as well as make sound independent decisions.

QUALIFICATIONS
-Strong verbal and written communication skills
-Bilingual in Spanish, preferred
-Bachelor’s Degree in social services or human services or equivalent experience in the human services field
-Thorough understanding of the landscape of health and human services resources
-Proficient in Microsoft and suites

PHYSICAL REQUIREMENTS
The employee is regularly required to communicate clearly and exchange accurate information interpersonally or through communication devices. Must be able to learn new systems and procedures, prepare and analyze data and figures, constantly operate a computer and other standard office machinery. The position requires occasional movement throughout the office to access files, papers, or move supplies weighing up to 25lbs. Must be able to remain in a stationary position 75% of the time.
