POSITION OVERVIEW

The Care Coordinator, Housing is responsible for addressing the diverse spectrum of housing needs
for families at risk of homelessness and housing insecurity in Shelby County. DTD presents real
solutions for the most vulnerable in our community by providing flexible funding, access to emergency
housing, transitional housing, and independent living (e.g., rapid rehousing). In partnership with the
Agency Relationship staff, generate and manage housing leads to grow the landlord network through
landlord visits and cultivation. Maintains detailed records on housing status, landlord recruitment,
housing inspections, and payments made on behalf of clients (security deposits, rent or mortgage,
utilities, legal fees, additional support for barriers to stabilization). The Care Coordinator positions are
integral to DTD. With a primary responsibility of working with clients to provide coordinated care by
developing, monitoring, and evaluating care plans, connecting clients to needed services, and
following up with clients and providers to assess progress toward client goals. In this unique

PRIMARY DUTIES AND RESPONSIBILITIES
● Clearly articulate the United Way of the Mid-South and Driving the Dream mission, goals and
value proposition to landlords and community stakeholders.
● Complete intake, interviewing, assessment, and referrals to identified service providers.

● Provides case management with a focus on crisis management, employment, housing, and other
needs.
● Process all referrals in a timely manner.
● Establish and maintain relationships with enrolled clients through monitoring and follow up as
determined by established policies and procedures.
● Provide weekly, monthly and quarterly reporting updates.
● Participate in all meetings, trainings, and professional development opportunities.
● Prepare, review, and submit reports within the required time frames.
● Actively generate and qualify new housing leads for families.
● Recruit new individuals/companies to landlord network, through ongoing landlord cultivation,
encouraging them to become “preferred landlords”.
● Maintain landlord relationships through ongoing communications, resolution of client issues, and
ongoing inspection to ensure properties are taken care of, and prompt customer service.
● Ensure adequate rental units are available for clients, in order to move families quickly from
shelter/homelessness into appropriate housing.
● Contact families within 24-48 hours upon receipt of referral to discuss housing programs, assess
housing barriers, and begin making a housing plan.
● Arrange transportation for families to view and approve possible housing placements if needed.
● Understand the needs of clients and be able to effectively advocate for housing needs.
● Provide ongoing advice, expertise, consultation and technical assistance on housing matters.
● Serves as a liaison between United Way of the Mid-South and other housing agencies, providers
and landlords/property managers on all matters related to housing.
● Perform administrative duties related to paperwork and documentation for all housing programs
and grants, ensuring full compliance related to housing.
● Perform other duties as assigned.


QUALIFICATIONS
● Bachelor’s degree in Communication, Education, Social Work, Public Administration or other
related field.
● At least three (3) years of experience providing social work, care coordination, case management,
etc. in human services.
● Experience working with adults in managing and supporting client's needs.
● Must be fluent and flexible with technology (i.e., strong computer skills, particularly with
spreadsheets, databases, and remote access).
● Familiarity and a broad understanding of the local services and ability to connect clients to these
services to meet their needs.
● Ability to work effectively in an outcomes-based environment with competing priorities.
● Demonstrated knowledge of case management standards.
● The ability to assess status of participants, plan and evaluate care plans, monitor and assess
participant progress and needs.

● A broad understanding of local services and the ability to link services to participant needs.
● Ability to effectively interface with a wide range of partner community agencies.
● Maintain confidentiality and objectivity when dealing with sensitive client issues.
● Demonstrated commitment to the field and a passion for serving under-served and under-
resourced individuals, families and/or communities; particularly people living in poverty.
● Desire to serve as an integral leader at an established community institution in the midst of a
unique period of change and transition.
● Not just tolerance for, but enjoyment of, working with a degree of ambiguity during this
entrepreneurial period of transformation and ability to help others successfully move through this
change.

SUPERVISORY RESPONSIBILITY
No direct supervisory responsibility.
 

PHYSICAL REQUIREMENTS

While performing the duties of this job the employee is regularly required to sit, talk and/or hear. The
employee is frequently required to use finger and hand motion, and occasionally to stand, walk and
reach with hands and arms. Physical requirements include climbing stairs and being able to carry, lift,
and, set up equipment and furniture weighing up to 25 lbs. Specific vision abilities required by this job
include close vision, distance vision, depth perception and ability to adjust focus.
